5.3.1 Patient referral message (REF)
Figure 3 shown in Item (i) is an overview of the AS 4700.6 V2.5 referral message (REF) and varies from HL7 V2.5 Chapter 11 and AS 4700.6—2006 V2.4 in the following ways:
(a) The PD1, IN1, ORC/RXO/RXR/RXC/RXE/RXR/RXC/RXA/RXR and PRB/GOL/PTH segments and segment blocks are optional Australian extensions to the HL7 V2.5 Chapter 11 Referral Message.
(b) The ORC segment is not in HL7 V2.5 Chapter 11, but is required to be included in the implementation of AS 4700.6 V2.5 when providing clinical requests and history segment group in order to express context of the subsequent OBR/OBX segments.
(c) The OBR/OBX segment group is optional in HL7 V2.5 Chapter 11 but is required within the clinical requests and history group, i.e. every ORC is required to be followed by an OBR. The IAM segment is not in HL7 V2.5 Chapter 11. IAM should be preferred but AL1 can be used if IAM is not, as it contains more comprehensive allergy information. AL1 is retained for backward compatibility only and may be removed from future versions of AS 4700.6.
(d) The sequence of PV1 and PV2 has been changed from HL7 V2.5 and the PV1/PV2 group is mandatory in the Australian Standard.
(e) The REL segment is a V2.6 segment from Chapter 12. It has been incorporated in this message to enable data items and segments to be related.
(f) SFT segment has been added in HL7 V2.5.
5.3.3 Referral response message (RRI)
Figure 4 shown in Item (a) is an overview of the referral response message (RRI) and consists of the segments as detailed. The RFI segment includes the referral status RF1-1 and the external referral identifier RF1-11.
This is to inform the originating referrer about the referral, and, where applicable, the allocated ID. It is not intended to convey any clinical findings from the referred to provider.
Reports, results, recommendations etc. from the originally referred to provider would be included in another REF message. The RRI message, at the minimum, consists of the following segments: MSH, MSA (ERR in the event of an error), RF1 and PID.
The remaining segments of the RRI message as per HL7 V2.5 are legal but should not be used in the Australian context.
